How to use it

A privacy-conscious team sheet for the minimum details a coach needs to organize attendance and reach a parent or guardian.

  1. Record the player's name, jersey number, preferred name, and attendance note.
  2. List one primary parent or guardian contact and a second contact only when the family supplies it.
  3. Add a short, clearly labeled section for information the league has asked the coach to carry, such as an allergy alert; do not collect details you do not need.
  4. Mark the date the sheet was last checked and remove outdated copies at season end.

Setup notes

Fill this in privately from the league's approved registration information. Store printed copies out of public view and follow the league's retention and privacy process.

Coach cues

  • Keep emergency information separate from a sideline handout.
  • Use a current date on every copy.
  • Never leave the sheet in a shared photo or public team post.
