How to use it
A simple chain-of-custody sheet for recording who took which equipment, when it is due back, and what needs repair.
- Give every bag, ball cart, or specialty item an inventory number before checkout.
- Record quantity and condition at pickup; use a short note for missing, cracked, or unsafe items.
- Capture the responsible adult, pickup date, expected return date, and signature.
- At return, count the same items in the same order and move repair needs to a follow-up list.
Setup notes
Keep the master inventory with the equipment manager. Bring only the current team's checkout page to the field.
Coach cues
- Count before signing, not from memory.
- Do not issue visibly damaged equipment for play.
- Set a named follow-up owner for every missing item.
