1. List players by name and jersey number, then mark attendance and any pre-game note.
2. Choose simple substitution checkpoints that fit the league's match format.
3. Use one row for each planned group and leave a column for actual entry and exit times.
4. Add a note for positions or roles to revisit, not a promise of equal minutes or a rule override.
5. Review the plan during natural stoppages and adjust for safety, fatigue, or the league's requirements.

Setup notes

Confirm the local substitution format before filling the sheet. Keep it on a clipboard with a pencil and a visible watch or game clock.
