- Choose a short list of observable behaviors for the sport and age group, such as ready position, effort, communication, or decision-making.
- Use the same scale and plain-language anchor for every player.
- Record one observed strength and one next practice focus; avoid medical, character, or unsupported conclusions.
- If multiple evaluators are involved, compare notes after each station rather than discussing a child in front of the group.
- Store completed forms according to the league's privacy process and share only with the intended audience.
Setup notes
Define the scale before evaluations begin. Use the form to plan instruction and grouping, not to make claims beyond the evaluator's role.
Coach cues
