- List the roster in jersey-number order and note any pre-game attendance changes.
- Divide the game into local-rule-appropriate shifts or checkpoints rather than promising exact minutes.
- Record who checks in, who checks out, and the team's current foul or timeout notes if useful.
- Leave a blank column for adjustments when the game pace or local rules require a change.
Setup notes
Prepare one coach copy and one assistant copy. Confirm the league's substitution and playing-time rules before using the grid.
Coach cues
- Call the next group early.
- Use jersey numbers and names together.
- Treat the sheet as a reminder, not a substitute for watching safety and matchups.
